Join an amazing team of professionals that are responsible for the day-to-day operations and activities related to clients' access to a Designated Living Option in Continuing Care for the Central Zone (Designated Supportive Living and Long Term Care). The Continuing Care Counsellor - Coordinator uses a collaborative approach to review all applications for Continuing Care Living Options, as well as manage the Central Zone wait list according to guidelines and policies in place. The coordinator will also act as a resource for facilities and community health professionals, revise priorities on a continuous basis and coordinate the placement of individuals in Designated Living Option spaces in the Central Zone.

Description:

As a Continuing Care Counsellor (CCC), you will be working closely with interdisciplinary professionals with a variety of backgrounds where you will provide a wide variety of coordination activities to support patients and their families to access Continuing Care living options. Using a collaborative approach, you will review applications, manage continuing care waitlists and coordinate placement processes. You will be guided by your education, experience, demonstrated skill level and AHS policies and procedures, while reflecting the shared vision and values of AHS.

Required Qualifications:

Active or eligible for registration with applicable professional association or college.

Additional Required Qualifications:

2 years of clinical experience. Working knowledge of the Meditech system. Comprehensive knowledge of gerontology. Knowledge of Continuing Care system. Case Management experience. Experience in completing Resident Assessment Instrument–Home Care. Experience in community care and home care.

Preferred Qualifications:

Bachelor's degree in the health care profession. Registered Nurse or Registered Psychiatric Nurse experience preferred.
