About our Client:

Cedarhurst Dementia Care Home is a small and secure residence for people living with dementia including Alzheimer’s. We offer care for a maximum of 26 residents. Each section in the home has six or seven residents, sharing their own kitchen and dining area, with a dedicated personal support worker. The strong caregiving support within each small group allows residents the comfort of their routines plus the opportunity to enjoy flexibility in their day-to-day activities.

Our goal is to provide a nurturing home-like environment, engaging residents through creative, stimulating and meaningful activities that suit each individual’s needs and abilities. Cedarhurst Dementia Care Home is different from most retirement homes and nursing homes in that we do not provide dementia care as an add-on to a more general business. Because we specialize in caring for those with dementia, and our home was built specifically for this population, we are ideally suited to manage their unique challenges.

Position Summary:

The Director of Life Care (DOLC) will be a Registered Nurse (RN), will work in collaboration with the Care Coordinators and will be responsible for clinical oversight of direct resident care.

The DOLC will ensure that activities support the Home’s mission, vision and values, while adhering to the Home’s policies and procedures. The DOLC will participate in the development and oversee the implementation of the evolution to joyful living, including, in accordance with the Home’s directive, be responsible for leading the implementation of all educational activities.

Responsibilities:

Clinical Care

  • Provide nursing care and clinical oversight of direct resident care within scope of practice with the CNO
  • Complete medication reconciliation and achieve regulatory compliance
  • In collaboration with General Manager and Care Coordinators, design and oversee implementation and evaluation of resident specific treatment strategies, including continence, pain, and responsive behaviours
  • Serve as primary contact for home physician
  • Liaise with external providers including medical specialists, pharmacy provider and other health care professionals regarding resident care
  • Design and manage processes for outbreaks
  • Work closely and communicate with Care Coordinators and General Manager to ensure smooth, effective daily communication of resident status and resident and family experiences
  • Mentor and coach the Care Coordinators, and, in collaboration with the Care Coordinators, mentor and coach the PSWs and nursing students

Administrative and other Responsibilities

  • Complete resident assessments for the eligibility of potential residents for move-in to the home
  • Develop assessments post move-in and ongoing resident assessments in collaboration with Care Coordinator and GM
  • Create a ‘Resident Living Profile’ for each Resident
  • Design and lead care conferences with families including external providers as appropriate
  • Maintain health care records/resident profiles to ensure compliance with RHRA regulations and quality of care for residents
  • Serve as back-up to the Care Coordinator for planned and unexpected absences
  • Design and manage processes for private caregivers as well as additional non clinical external providers (e.g. foot care, hair dresser)

Qualifications:

  • Registered Nurse (RN) with current certificate of competence with the College of Nurses of Ontario
  • Minimum of 3 years of demonstrated expertise in working with people living with dementia, preferably in retirement or long term care setting
  • Demonstrate experience with having provided education in respect of care of older adults
  • Knowledge of CNO Standards, Best Practices in Nursing and Dementia Care, legislative requirements including the Retirement Homes Act
  • Completion of P.I.E.C.E.S. and/or Gentle Persuasive Approaches in dementia care
