JOB SUMMARY
Reporting to the Clinical Manager, the Registered Social Worker is responsible for the provision of short-term counselling, case management, systems navigation, and referral triage to support Community Health Centre clients in collaboration with the clinical team. A key focus will be supporting complex clients with achieving optimal psychosocial functioning and optimizing access to vital Social Determinants of Health.

Education:

  • Master’s degree in Social Work (preferred).
  • Bachelor’s degree in Social Work (minimum requirement)
  • Registered and in good standing with Ontario College of Social Workers and Social Service Workers (OCSWSSW)

Duties and Responsibilities:

  • Provides short term counselling and emotional/mental health support, to address client psycho-social needs, working towards achieving established therapeutic goals until access to a community resource can be established through referral.
  • Triage referrals to appropriate supports for complex mental health disorders, addictions and/or referrals for additional treatment.
  • Establish therapeutic relationships with clients, their family members/caregivers/SDMs in order to build trust and effectively deliver appropriate care services.
  • Develops client treatment plan in collaboration with the client and primary care team.
  • Monitors client progress and adjust goals and treatment plan accordingly.
  • Educates and supports clients, their family members/caregivers/ substitute decision makers (SDMs) with challenges, as well as provide strategies to improve functioning, prevent unnecessary deterioration and promote independence.
  • Manages service delivery in person, by telephone, OTN, home visiting, and written correspondence.
  • Advocates for clients as warranted, and facilitates connections between clients and community supports, to assist clients in accessing required resources.
  • Act as a resource and advocate for clients and families with regards to interpreting. information, supporting decisions and client safety and care.
  • Provides system navigation and case management by facilitating connections between clients and community supports, to assist clients in accessing required resources.
  • Uses sound professional judgment and acts in accordance with all SGBCHC policies and procedures and standards of practice set forth by the Ontario College of Social Workers
  • Maintains professional competence and knowledge of current practices by participating in professional development and clinical supervision.
  • Works as a member of a multi-disciplinary team in collaboration with community partners for the purpose of consolidating and coordinating client treatment.
  • Follows established SGBCHC and professional college standards for documentation.
  • Protect client confidentiality in accordance with PHIPA, CHC policies and standards established by the College of Social Workers and Social Service Workers (OCSWSSW)
  • Other duties as assigned.

Knowledge/Experience:

  • Relevant social work experience in a community/primary health care environment.
  • Comprehensive and current knowledge of social work and psychotherapeutic theories and interventions.
  • Excellent knowledge of community resources including but not limited to treatment centres, addiction services, and social services.
  • Maintain a comprehensive understanding of crisis management and the impact of trauma.
  • Comprehensive knowledge of client centered addiction interventions.
  • Good understanding of the roles of other health care professionals and complementary therapies.
  • Knowledge of funding agencies available to assist clients in accessing needed support.
  • Advanced experience with computer software, electronic medical records (EMR) and Microsoft Office applications.
  • Experience working with marginalized populations with mental illness and addictions, trauma, (e.g. newcomers, immigrants, seniors, youth, 2S-LGTBQ+, homeless, individuals living in isolation and in poverty who are at risk).

  • Strong interpersonal skills to work with diverse client groups with varying levels of comprehension and language capability, demonstrating empathy and compassion.
  • Effective listening, observation, and facilitation skills to effectively assess clients and provide appropriate treatment and consultation.
  • Strong mediation, negotiation, and conflict resolution skills to respond or assist in crisis situations.
  • Ability to think critically, and be innovative to create/modify strategy or intervention techniques to suit the individual needs of clients using a health equity lens.
  • Demonstrated ability to organize, prioritize and multitask efficiently.
  • Demonstrated ability to analyze and make independent decisions using a health equity lens.
  • Ability to communicate information effectively through a variety of means including meetings, reports, letters, and presentations.
  • Computer literacy
  • Ability to deal with demanding interpersonal situations and respond with good judgment, compassion, while continuing to foster healthy relationships.
  • Demonstrated ability to work as a member of a multi-disciplinary health services team demonstrating a positive attitude and commitment to inter professional practice.
  • Ability to understand and practice infection and prevention control policies, adhering to pandemic precautions in the workplace.
  • Demonstrated ability to be flexible with the ability to adapt quickly to the changing needs of the programs and operation with a positive attitude.
  • Valid Ontario driver’s license and use of own vehicle for CHC business

Health and Safety Commitment:

  • Comply with the Occupational Health and Safety Act (OHSA), its regulations and all CHC health and safety policies and procedures
  • Use or wear any protective equipment, device or clothing required by their supervisor
  • Report to their supervisor any known missing or defective protective equipment or protective device that may be dangerous
  • Report any known workplace hazard to their supervisor/manager
  • Report any known violation of the OHSA or regulations to their supervisor/manager
  • Not use or operate any equipment (includes motor vehicle if required for work) or work in a way that may endanger themselves or any other worker
  • Attend all health and safety training as directed by their supervisor/manager and apply this knowledge to their job
  • Report workplace accommodations when required.
