Detailed Overview:
The Social Worker works as part of a multi-disciplinary team using a collaborative approach ensuring continuity of care with community resources. The Social Worker conducts comprehensive social work and psycho-social assessments of clients and families using a variety of modalities and in accordance with professional standards and clinical policies and practices. The Social Worker provides direct care to clients and families in order to assess needs, support and link clients with community services and participates in the evaluation of the program to ensure maintenance of standards of care. The Social Worker develops, coordinates and delivers inservices, presentations and psycho-educational groups.

Incumbent will also be required to provide services in Mental Health & Substance Use-CRESST (Fraser East).
Responsibilities:
  • Provides direct care to clients through formation of psychosocial assessment and treatment plan, gathering information about the client and client family with respect to social, emotional, cultural, environmental and financial situation, and utilizing a variety of treatment modalities to provide treatment; documents care plan in accordance with established standards and procedures.
  • Liaises with primary physician, team members, and other staff and referral sources regarding client care issues.
  • Liaises with appropriate health and community resources in order to maintain a strong inter-relationship with the hospital and/or community resources and to provide the relevant information to facilitate service linkages as appropriate.
  • Provides consultation and education to lay and professional community members or agencies to address the needs of persons with serious and persistent mental illness.
  • Organizes and implements psycho education groups.
  • Participates in the orientation and collegial sharing of information to staff and students, as well as, education sessions for community members and/or agencies. Develops, coordinates and delivers inservices, presentations, and/or training for other health professionals on topics related to mental health issues and services by methods such as collaborating with members of the interdisciplinary team, determining relevant content, conducting literature searches and planning and coordinating seminars.
  • Facilitates clients’ utilization of appropriate community services and rehabilitative programs by performing duties such as researching available services, liaising with these agencies, providing related advice and consultation to clients and providing referrals to appropriate community treatment alternatives and services.
  • Facilitates the process of admission when required by preparing the client and providing pertinent information to the Psychiatric Liaison Nurse and physicians in the Emergency Department.
  • Provides support and direction to clients by assessing their adjustment and ability to perform routine life activities, including managing money and taking medication regularly. Provides referrals for follow up care and services for clients as required, including providing referrals to other healthcare professionals, such as Recreation Therapists, Occupational Therapists, and Psychiatrists.
  • Transports residents to programs, appointments, services and other activities; books client trips, alters schedules, arranges time for pickup and dropping off of residents.
  • Maintains client records and statistics; prepare reports and completes forms as required in accordance with FHA policies and standards, including clinical record standards of Mental Health Services. Obtains consent forms for release of client information.
  • Advocates on behalf of clients by performing duties such as providing clients with information about appeal processes, providing information about clients to relevant Ministries, and assisting clients to complete forms. Acts as a resource for clients and their families.
  • Provides education to clients, their families, and staff of residential facilities and other agencies regarding community mental health programs and issues.
  • Participates in quality improvement and risk management activities for assigned area by performing duties such as participating in quality initiatives and providing information and input from the Manager regarding programs and initiatives.
  • Participates in ongoing education to maintain current and relevant practice, including clinical education, as well as, maintaining current knowledge of Federal and Provisional legislation and amendments that impact mental health client care and service delivery.
  • Participates in clinical research and program evaluation activities relevant to the needs of clients, families, hospitals and the community by providing information as requested.
  • Performs other related duties as assigned.
Qualifications:
Education and Experience

Current full registration with the BC College of Social Workers.

Degree in Social Work from an accredited school of Social Work. Two (2) years' recent related experience in mental health treatment setting or an equivalent combination of social work education training and experience.

Valid Class 5 BC Drivers License and access to personal vehicle with appropriate business insurance coverage.

Skills and Abilities
  • Comprehensive knowledge of mental assessments and screenings, systems theory and an ability to apply this theory to practice Social Work assessments and intervention.
  • Comprehensive knowledge of psychiatric disorders including the DSM IV and of psychosocial issues related to these disorders.
  • Ability to plan, implement, organize, prioritize, problem solve and identify risk factors.
  • Ability to establish and maintain effective working relationships with other health practitioners, as well as, clients, family members, community resource agencies and the public.
  • Physical ability to perform the duties of the position.
  • Demonstrated ability to facilitate couple and family meetings.
  • Demonstrated ability to develop, coordinate and deliver in-services, presentations and psycho-education groups.
  • Demonstrated ability to educate and provide consultation to patients, families and healthcare professionals.
  • Demonstrated ability to assist crisis prone individuals.
  • Demonstrated ability to communicate effectively both verbally and in writing with clients, families, staff and community agency personnel.
  • Ability to effectively deal with conflict situations.
  • Demonstrated ability to operate related equipment.
