Case Manager – Addictions Supportive Housing

Hours of Work: 35 hours per week

Purpose of the Position: The Case Manager provides assistance on a day-to-day basis through a continuum of functions that can include: initial and ongoing assessment, treatment planning and ongoing adjustments as required, community treatment, referral and integration of required services, life skills enhancement, advocacy, personal care coaching, self-efficacy promotion, and collaboration with other service providers. The Case
Manager will assist service users to maintain stability in the community through intensive case management services designed to address problematic substance use/concurrent disorders as well as their accommodations according to their needs and choices. The Case Manager will facilitate help to service users with problematic substance use issue (s) who are currently homeless, at risk of becoming homeless or who are being released from a community setting (e.g. hospital) and require housing.

Education and Qualifications:

Bachelor’s degree in a human service/health science discipline relevant to the position. Equivalency may be considered for a combination of educational experiences. Examples of equivalency include: o Addictions: Diploma in human services/health sciences PLUS post-diploma specialization in addictions o Mental Health related diploma with relevant work experience PLUS registration with a regulated health professional college

Broad knowledge of addictions treatments and explanatory theories, homelessness, mental illness, and community resources

Experience in providing individual, family, and group therapy and supporting persons with complex needs in varying stages of change

  • Experience in providing practical assistance, basic life skills counselling, supportive counselling, crisis intervention, advocacy, and group work, including mediation and conflict resolution

Ability to identify issues (practical, individual and interpersonal) and use related problem solving skills

  • Ability to work effectively under pressure or deadlines, with constant interruptions or unpredictability
  • Ability to work autonomously as well as cooperatively within a team environment in a rural setting
  • Excellent communication, organization and interpersonal skills
  • Working knowledge of Microsoft Windows, the Internet, and data management software
  • A willingness to serve clients within the communities of the District of Muskoka and Parry Sound
  • A valid Ontario G or G2 driver’s license, a safe and reliable means of transportation, and appropriate insurance for work purposes on your vehicle with a minimum of $1,000,000 in liability coverage.
  • Bilingual in French/English language considered an asset
  • Work schedule may include days/evenings/weekends and statutory holidays.

Salary scale: $28.41to $37.20 per hour commensurate with qualifications and experience
Benefits include: Comprehensive benefit plan and enrollment in a Registered Retirement plan with HOOPP where contributions start on date of hire.
