The Toronto Grace Health Centre (TGHC) provides specialized care and services to those individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation, Palliative Care, and Transitional Care. The care and services provided at TGHC help facilitate the patient flow through the health care system and ensures recovery to those individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities. TGHC operates in 3 sites – the Main site (Church& Bloor St), Harbour Lights (Queen & Jarvis Street) houses and a Specialized Care Centre (Victoria Park & McNicoll)

Position Summary and Responsibility:

The Charge Nurse provides excellent patient and family centered nursing care in complex patient environment in a culturally sensitive manner. Establishes caring, therapeutic relationships with patients and their families to attain or maintain optimal functioning by promoting health and/or treating health conditions through supportive, preventative, therapeutic, palliative and rehabilitative means.

  • Coordination of daily administrative duties including nursing schedules in keeping with relevant clauses from the collective agreements. This will usually occur working in close collaboration with the unit clerk
  • Continually reviews the patient assignments utilizing the three factors framework (Patients, Staff and Environmental factors) to inform patient care activities on the unit for each shift
  • Supervises and supports the professional practice of nursing staff on the unit
  • Acts as the contact person on the unit for patient flow & operational inquires including the coordination of patients admissions, transfers (internal and external), and discharges to ensure a seamless transition for patients and families
  • Facilitates the delivery and communication of appropriate patient care planning activities with the interprofessional team to ensure continuity of care for all patients
  • Promotes and facilitates implementation of patient safety related strategies with the interprofessional team such daily patient safety huddles, hourly rounding and clinical patient rounds
  • Monitors the quality of patient care through daily patient huddles and monitoring of key quality metrics (for example: Falls, Pressure Injuries and Medication errors)
  • Assists in the development of effective interpersonal relationships among interprofessional team through coaching, mentoring and facilitating difficult conversations
  • Reports clinical/performance issues to the Patient Care Manager including potential solutions or options for resolution.
  • Serves as the main point of contact for infection prevention and control practices and activities on the unit
  • Acts as a positive role model and mentor for all members of the inter-professional team
  • Supports all new education and professional development initiatives
  • Promotes positive patient relationships and addresses issues in a timely fashion
  • Some additional aspects of the Charge Nurse role also include:

Liaising with doctors and administrators
Communicating any protocol changes to staff
Ensuring compliance with all health and safety regulations
Identifying issues or emergencies and responding in a calm and efficient manner
demonstrating good problem solving skills
Regularly complete chart reviews and audits to ensure adherence and compliance
to standards of care
Supports the College of Nurses Standards of Practice and the Quality Assurance
Program

Qualifications:

· Registered Nurse with excellent standing with the College of Nurses and a Bachelor of Science in nursing

· Minimum 2 years of mental health nursing experience required : in acute care facility or community setting

· Non-violent Crisis intervention certificate an asset

· Ability to conduct a nursing mental health status evaluation

· Experience with assessment, monitoring and evaluation of patients with mental health illness and addiction disorders

· Knowledge of community resources to support patients with mental health illness and addiction disorders

· Familiarity with the clinical operations of an in-patient unit

· Ability to complete MDS assessments

· Skills and abilities necessary to provide guidance and assistance as needed on the clinical unit

· Demonstrated respect and support for diversity within and among our patients, families and teams

· Recent experience in mentoring and preceptoring required

· Advanced knowledge of hospital safety practices, standards, policies and procedures, reporting structure and emergency measures

· Exceptional interpersonal and communication skills (verbal and written), including conflict resolution, negotiation, ability to have difficult conversations.

· Ability to model leadership behaviours and create a positive work environment where staff are motivated to do their best and strive for continuous improvement

· Exceptional critical thinking and organizational skills

· Basic Cardiac Life Support Certificate required

· Proficient in Microsoft Office, Outlook and medical software systems.

· Excellent performance and attendance required
