Nestled in the heart of the beautiful rural community of Wiarton is Gateway Haven Long Term Care Home. Alongside the picturesque Colpoys Bay in Wiarton, 100 residents call Gateway Haven Home. Built in 2003, the home has many wonderful features that contribute to the health and well being of our residents. The unique "C" shaped building allows each resident to have a view from their room whether it is a view of our beautiful park, surrounding neighbourhood or a picturesque view of Georgian Bay.
The success of the operation of Bruce County's Long Term Care Homes is built on the foundation that this is a true 'home' for all its Residents. About the Job

This is a great opportunity to join our team of passionate professionals in a role that provides clinical expertise, mentorship and guidance to nursing staff in the provision of exceptional resident care. Champion best practice as a leader and educator, support and enhance high level problem solving, diagnostic, and decision-making skills, all while putting "Residents First Every Day…Every Way."

Reporting to the Director of Nursing, the Clinical Care Coordinator coordinates identified nursing leadership responsibilities and clinical competency maintenance in support of resident care and the nursing staff. The Coordinator will engage as clinical expert and acts as a role model for the nursing team. The Clinical Care Coordinator works a varied schedule which includes alternate weekends to provide appropriate leadership and support to the nursing team and residents. With notice, the CCC schedule will vary at times to accommodate monthly orientation and other needs.

Schedule
Two week schedule rotation of day shifts (6:00am - 2:00pm) and evening shifts (2:00pm - 10:00pm).

Salary
Wage Range: $36.73 - $52.61 per hour based on the ONA collective agreement.

Requirements

The successful candidate will meet the following requirements:
  • Current registration with the College of Nurses of Ontario
  • Certificate in Gerontology, a Bachelor’s degree in Nursing (BSN) or a Certificate in Clinical Administration
  • Minimum of seven (7) years of experience as a registered nurse, with a minimum of five (5) years in geriatric nursing or a long term care setting
  • Proven experience in coordinating and supervising others
  • Competent and skilled with phlebotomy.
  • Excellent interpersonal and communication skills and the ability to work inter-dependently
  • Demonstrate initiative, flexibility, good organizational skills and good supervisory skills
  • Comfortable and highly skilled with all documentation, including documentation of risk management.
  • Resident/family oriented and able to act as an advocate of resident rights
  • Must be knowledgeable in all emergency measures, be prepared to implement and respond within an appropriate time frame
  • Crisis-intervention skills
  • Must be available for shift work in the event of an emergency which may include being on call.
  • Good physical and mental health and a professional attitude
  • Clear criminal background check (vulnerable sector screening)
  • Two step tuberculosis skin test
  • Proof of full vaccination against COVID-19 (3 doses)
