We are looking for Homeward Bound (HB) Coordinators, who will work in collaboration with the Program Manager of Homeward Bound program. The successful candidates will be responsible for the coordination and implementation of all aspects of this community program, designed to facilitate a smooth transition from hospital to home and ensures that community support services are in place for clients. The HB Coordinators will work closely with front-line staff and are required to be knowledgeable of the other CH&W programs, to at times be required to assist in day-to-day operations within the Chatham-Kent region.


In this role, you will:

  • Participate in the creation, maintenance and updating of client discharge plans that establish program goals ad objectives for each client
  • Research and implement strategies to address discharge barriers to support client safe recovery at home
  • Approve referrals, complete intakes, and determine internal and external care coordination
  • Schedule requests for service/transportation
  • Follow up with HB clients two weeks after discharge from program, and complete client satisfaction surveys
  • Monitor and maintain excellent customer service and quality management in service delivery
  • Assist Management with development and implementation of the annual program work plan
  • Support Management in the recruitment, training, coaching, and evaluation of staff and volunteers associated with the program
  • Assist with preparing and monitoring program specific budgets, complete program statistical reports and prepare progress reports
  • Monitor program operational needs and expenses
  • Directly carry out client transitions when short staffed
  • Liaise with hospitals, community agencies and organizations regarding related services and initiatives to foster mutual understanding, coordination, and partnership which will enhance the client experience
  • Follow all policies and procedures as required to maintain a healthy and safe working environment
  • Perform other duties as required, in consultation with Management

What we are looking for:

  • A minimum of one to three years community college diploma in Gerontology, Social Services or related area and between one to three years job related experience or an equivalent combination of education and experience
  • Supervisory or project management experience, including the ability to lead, motivate, and evaluate
  • Ability to develop, implement, and evaluate program plans
  • Knowledge of community support services
  • Experience working with clients and volunteers
  • Scheduling experience is an asset
  • Excellent interpersonal skills, including communication and presentation skills
  • Ability to work independently, prioritize, multi-task, and achieve successful results
  • A valid Driver’s License and access to a reliable vehicle – a satisfactory Driver’s Abstract is required
  • Criminal background check and Vulnerable Sector check will be required
  • Standard First Aid / CPR certification

Working Conditions:

  • This position will be resuming in-home intakes in the near future. Once resumed, the successful candidates will be required to travel in the community to client homes to complete intake assessments using a RC vehicle.
  • Presence at the CRC office in Chatham-Kent will be required. The office environments are clean and comfortable.
  • Availability in the Chatham-Kent region and willingness to travel to the Sarnia branch (minimum 1 day per week) to support the Sarnia Homeward Program will be preferred
