Purpose:

The clinical Dietitian as a member of the multi-disciplinary care team shall ensure quality care to the residents of PEI Atlantic Baptist Homes Inc. This role shall require:

Assessment, planning, implementation and evaluation of individual residents nutritional Care plans.

The provision of diet instruction and counseling to the resident, care staff and family as the Dietitian intervenes to help the individual adjust to changes in lifestyle, physical and emotional well being, social atmosphere and living environment.

The interpretation of care plans to members of the care team, Dietary staff, Food Service Manager to ensure continuity of care.

To direct the nutrition management and therapeutic aspects of the Dietary Department by assisting in the identification of goals and objectives, and recommending ways to meet these through the establishment and evaluation of programs, recipe development, education of staff and policy & procedure development.

The Dietitian shall also act as a consultant for the other facilities operated by the Atlantic Baptist Senior Citizens’ Homes Inc., as required.

Duties:

  • Visit residents within 3 weeks of admission to assess nutrition needs and preferences. Respond to referrals from physicians/NP, case managers, Resident Care Coordinator reports from the Operations Manager and other members of the health care team as well as requests from residents and family.
  • Liase with all of the above including partnerships with other facilities (transfers from other Homes, Extra Mural Hospital Services team members-speech language pathologists, physiotherapists, occupational therapist, dietitians; medical staff from the Moncton and Georges Dumont Hospitals. (Inpatient Dietitian & Dialysis Unit Dietitian)
  • Reviews daily 24 hour.
  • Performs nutritional assessments, diet histories, calorie counts, feeding assessments, and restorative feeding interventions. Detail strategies and interventions for nutritional care of specific residents and reviews care teams’ ability to follow up.
  • Designs meal patterns and makes provisions for nutritionally adequate and appropriate foods that are provided within the Home’s meal service system.
  • Monitors and evaluates the adequacy of nutritional intake of residents, care plans, special diets, tube feedings, nutritional supplements and routine weight checks of residents.
  • Attends meetings specific to the care of residents (i.e. Residents Care Plan-Health Team Meetings, Multidisciplinary Conferences) and provides nutritional intervention as required and updates Care Plan.
  • Maintains dietary profile as part of each resident’s complete care plan in the computerized Resident Information Management System.
  • Maintain up-to-date records on the resident’s chart according to established procedures and record keeping systems.
  • Support Food Safety Standards throughout facility.Assesses and approves Cyclic Menu to ensure meals and snacks meet Canada’s Food Guide to Healthy Eating for senior population.
  • Maintains necessary records in the Dietary Department including copies of the RIM dietary care plan, reviews resident dietary requisition forms, and directs Dietary Staff as required in completing individual resident tray cards and Dining Room diet sheets.
  • Provides input to the Director of Care, Nurse Manager and Food Service Manager in regards to nutrition services as it relates to specific programs, staff education, policies and procedures, menu development, and short- and long-term planning.
  • As Education Coordinator, works with Managers in identifying and supporting educational opportunities for staff, based on Best Practice. Acts as Chairperson of the Quality/Worklife Education Committee which focuses on staff well-being and education. Prepares monthly report for Quality Management.
  • Serves as a member of the Infection Control Committee, Quality Management Team, Occupational Health and Safety Committee.
  • Evaluates and coordinates the protocol for residents in the Home requiring tube feeding and develops their individual plan of care including the type of formula recommended, the feeding pump and tubing required, the delivery of the feeding. Contacts sales representatives and collaborates and orders from supplier’s necessary tube feeding formulas/supplies.
  • In collaboration with Nursing, develop tube feeding policies, procedures and coordinates staff training as required.
  • Nutrition intervention is utilized whenever possible to minimize residents’ use of medications.
  • Performs other related duties as assigned by the Administrator and the Director of Care.
  • Consults and visits other facilities of the ABSCHI as required.

JOB SUMMARY:

Under the general direction of the administration of the Nursing Home, the Dietitian participates actively on the quality management team. The Dietitian works in collaboration with the Operations Manager in order to maintain service at the highest possible level of efficiency as outlined by department and nursing home policies.

The Dietitian assists in the co-ordination of the Home’s staff education by participating on the Education Committee in the role of Quality Work life/Education Chairperson.

Nutrition is a basic care need of every individual. It is the responsibility of the Home to provide residents with the food and fluids necessary to meet nutritional needs, support symptom management of chronic diseases and enhance quality of life. The dietitian through expertise and a member of the multidisciplinary team supports quality nutritional care.

QUALIFICATIONS:

Professional Dietitian – Bachelor of Science Degree in Foods and Nutrition; graduate from a Dietitian’s of Canada approved and a member of Dietitians of Canada.

Minimum of three years current experience in the dietetic field, with preferably varied responsibilities such as therapeutics, including geriatric nutrition, staff education and food service administration is preferable.
