Edmonton Zone Continuing Care provides an integrated system of health and personal support services to clients living in their own home, in a supportive living setting or in a long term care setting. Working in this program, you will have the opportunity to collaborate with an interdisciplinary team of professionals providing support and care to clients of all ages. Within this advanced model of care, you will work as a partnership with a common goal of clients achieving and maintaining their health, well-being, and independence. The Health Care Aide provides personal care and support to clients in a community setting. Work is assigned according to an established plan of care, in collaboration with the client, family and members of the interdisciplinary team. The Health Care Aide is supervised by a regulated health professional and may perform assigned Restricted Activities under specific circumstances. Documentation of pertinent information and care provision is required, as is participation in relevant in-service education. The Health Care Aide provides patient care services as an unregulated worker under the supervision of a Registered Nurse (RN), Registered Psychiatric Nurse (RPN), or Licensed Practical Nurse (LPN). This position will be based out of the Morinville office but may provide support to home care's Suburban/Rural West team, which may include travel to the other home care network offices, as required.

Description:

As a Health Care Aide (HCA), you are responsible for providing personal assistance, supporting activities of daily living and providing comfort and support services to patients (clients, residents) who require short-term assistance or ongoing support. You will work closely with patients, families, and caregivers spanning the continuum of care in home, community or health care facilities. You will play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.

Required Qualifications:

Active directory status on the Alberta Health Care Aide Directory. Must provide a confirmation letter as proof of enrolment from the Directory at https://www.albertahcadirectory.com/health-care-aides/confirmation/. Nursing students currently enrolled in an approved Alberta nursing program who have successfully completed a minimum of 100 hours of practicum; and are either in their 1st year of practical nursing program; or 2nd year of baccalaureate nursing or psychiatric nursing program will be considered. Must attest to this compliance and provide proof of qualification.

Additional Required Qualifications:

Completion of a HCA Certificate from an Accredited College/Program. The Ability to apply Health and Safety principles in delivering care to clients in the community, to work effectively and cooperatively with others and to set priorities by organizing and scheduling work-related activities is essential. BCLS-HCP is required. The successful candidate will work independently in clients’ homes, which could include single family dwellings, condominiums, and/or walk-up apartments. Therefore, the successful candidate must be able to work in different settings and environments (home environments and weather conditions), as well as be able to lift and carrying medical bags, which weigh approximately 25-30 lbs.

Preferred Qualifications:

Previous Community Care and/or geriatric experience. Previous experience in the health care field and work with older or chronically ill individuals. A Medication Assistance Certificate or prior experience in Medication Assistance.
