POSITION SUMMARY:

Reporting to the Manager, Practice and Compliance Supports responsible for Long Term Care, the Infection Control Practitioner will be a resident safety expert who leads the planning, development, implementation, coordination, and evaluation of system-wide improvements in infection prevention and serve as a role model to promote the goals of the IPAC program within the context of the vision and mission of the multi-disciplinary home team at the Lakeridge Gardens 320 bed long term care home built on the Ajax-Pickering hospital site in the delivery of excellent care for residents.

Having opened in early 2022, this new home is part of the government's plan to improve long-term care delivery and capacity in Ontario and in the Durham Region. This state-of-the-art long-term care home sets a new bar for seniors' care, with dining and outdoor space on every floor, a large courtyard, onsite diabetes care and other important innovative features.

Just as Lakeridge Health is a beacon in the Durham community, the new long-term care home is integral to managing and delivering on the needs and expectations of seniors in the Durham community and Ontario.

DUTIES & RESPONSIBILITIES:

  • To plan, direct, develop, coordinate, implement and evaluate resident care
  • To develop and maintain a relationship with the local Public Health, laboratory representative and IPAC leads at the regional and national level
  • To ensure resident satisfaction and continuous quality improvement initiatives are promoted
  • To implement Lakeridge's IPAC policies and procedures and participate in the development, assessment, planning, implementation and evaluation of the Home's IPAC Program
  • To participate in quality initiatives, perform and/or coordinate audits on a regular basis and prepare action plans to ensure adherence to IPAC standards and recommend improvements
  • To ensure surveillance is completed, using the IPAC app, by the nurse/designate on the shift of symptom onset, appropriate interventions implemented;
  • To identify and report infections and/or outbreaks, and monitor, investigate and follow-up infections and outbreaks and identify areas of improvement
  • To maintain a liaison with hospital, medical staff, community and health care related disciplines and partner to ensure the most effective provision of care delivery in the home
  • To oversee the financial and human resource management aspect of the IPAC function within the home
  • To ensure quality care and a safe, effective and efficient work environment
  • To coordinate and provide ongoing education to staff on the principles and practices of infection prevention and control and the new emerging infectious diseases and organisms
  • To ensure the availability and maintenance of IPAC supplies and equipment to meet the needs of the home in a fiscally responsible manner
  • To complete all other duties as assigned

QUALIFICATIONS:

  • RN/RPN degree or diploma.
  • Bachelor's degree in other healthcare related field, e.g., Microbiology, Epidemiology, Public Health, Health Science, Medicine or Occupational Health.
  • Current Certification in Infection Control (CIC).
  • Minimum of one (1) year of experience working as a healthcare professional in the long-term care sector
  • Minimum of three (3) years of experience working in a managerial or supervisory capacity in a health care setting
  • Current CPR designation is required
  • A minimum of 3 to 5 years of experience in infection control is required.
  • Previous experience in long-term care as well as knowledge of applicable legislation is an asset.
  • Experience and proficiency in the following: resident care practice, microbiology and infectious diseases, epidemiology and Biostatistics and outbreak response.
  • Demonstrated leadership and communication skills
  • Must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past 6 months;

  • Proven good attendance record with capability of maintaining this same standard;
