We are seeking a dynamic, highly motivated Life Skills Coach who demonstrates our values of compassion, accountability, responsiveness and empowerment; to our clients, families and each other.


The Life Skills Coach will be reporting to the Supervisor of Hamilton Housing Services, this position will assist in supporting individuals with complex needs who have experienced chronic homelessness with community inclusion, life skill development and the transition to their home. The Life Skills Coach works with the community and participants to provide hands-on assistance with the implementation of the individualized support plan as developed by the Intensive Case Manager and participant. This position will assist clients with their housing plans by developing creative, individualized approaches to empower clients and prepare them to succeed in securing and maintaining housing. The Life Skills Coach will also develop an instrumental Activities of Daily Living Manual that will be used as a tool and resource for the homelessness service sector. Services are based on a client-centred, residential harm reduction, housing first approach to service delivery.


This position is a unionized, temporary, full-time position. The rate of pay is $22.31 per hour. Temporary contract until March 31, 2024.


Key Responsibilities:


  • Conducts regular visits with participants in their homes or shelter to provide supports specific to life skills, daily living skills, conflict resolution, crisis intervention and prevention, budgeting assistance, financial literacy, community integration and community building, grocery shopping, cleaning, etc.
  • Assesses and screens participant’s/families’ needs for services to support their transition to stable housing and optimize their capacity to sustain housing using an acuity tool and other resources and referrals.
  • Assist participants with their move into independent accommodations and provide neighbourhood orientation.
  • Maintain regular, effective communication with social housing providers, administrators, community partners and private landlords, to ensure an integrated approach that will provide the best support possible for clients to maintain housing.
  • Develops and maintains Activities of Daily Living Manual
  • Implements risk assessment and crisis safety plans to ensure safety of individual and staff.
  • Report to appropriate child welfare agency (CAS) if child abuse and/or neglect is suspected or witnessed.
  • Maintain appropriate and accurate record keeping and reporting practice, as per expectations of Wesley
  • Other duties as assigned.

What you will bring:


  • Post-secondary degree/diploma in Human Services or related field
  • Registration and member in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) is required
  • Experience working with adults and families with mental health and addictions issues and the homeless and providing non-judgmental supports to them.
  • Must be able to work flexible shifts based on client need
  • Must be able to meet travel expectations of the position; a valid Driver’s license and access to a vehicle is a definite asset

As a condition of employment, the successful candidate is required to submit proof of full COVID-19 vaccination to Human Resources. In the event that successful candidates are unable to get their COVID-19 vaccination, completion of an educational session and weekly antigen testing to provide a negative result will be required.
