Edmonton Zone Continuing Care provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services and Palliative Care. The Manager, Community Care Access (CCA) is responsible for providing the direction, team leadership and organization over the activities of the CCA program within Continuing Care-Edmonton Zone: Manages daily operations of staff, monitoring the dynamic workload challenges, ensuring the efficient use of HR in providing the most effective delivery services. Promotes an interdisciplinary team culture in a 24/7/365 service delivery environment and provides a client-focused role model for staff. Collaborates with Program Manager to manage recruitment and staffing need, monitor operational issues, brings forward concerns and recommends solutions. Leads CCA team by providing guidelines and process information impacting nursing tele-practice, clinical and case management practice. Assists with complex case management. Implements and supports new or modified approaches, practices and processes. Develops staffing rotations, allocation of resources, ensuring compliance to applicable collective agreements. Supports staff to identify learning needs and facilitates continuing learning opportunities.

Description:

Resource Management: Manages the day-to-day operations of professional staff at the operations level monitoring the dynamic workload challenges and its effect on budget. Service Delivery: Fosters client centered care, recognizing unique client situations and complexity of care delivery in the community. Monitors effectiveness of contracted services provided by home support agencies and housing operators. Ensures services are delivered in accordance with relevant legislation and Alberta Health Services policies and protocols. Collaborates with staff to resolve complex client/family issues/risk issues. Reports issues to the Program Manager that impact, or have the potential to impact, the network/ program area or the zone. Makes operational decisions within the assigned area of responsibility based on critical review of clinical information, workload data and fiscal resources. Coordination and Communication: Promotes internal and external partnerships enhancing effective communication related to areas of responsibility. Staff Development: Fosters an atmosphere conducive to encouraging and meeting staff development needs and student learning experiences. Attendance and Performance Management: Monitors and documents staff attendance and performance. Provides immediate and constructive feedback while working with the staff to facilitate ongoing quality performance. Recruitment and Orientation: Participates in the recruitment process, from identifying the need to facilitating/providing orientation to newly hired staff.

Required Qualifications:

Bachelor’s degree in health-related field required (master's preferred). An equivalent combination of experience and education will be considered. A minimum of 5 years of clinical practice or health related experience required. Previous management experience in a health-related field is required. A valid driver's license and a vehicle are required. Computer literacy in Microsoft Office (Word, Outlook), ePeople/Recruitment Management System, and Environment for Scheduling Personnel (ESP) is required. Annual membership in good standing with an accredited professional association is required.

Preferred Qualifications:

Masters in health-related field preferred. Current community care preferred. Experience with Meditech is an asset. Effective communicator with strong organization skills, influential skills, coaching and teaching skills, client focused and commitment to providing quality services. Knowledge of community resources is an asset.
