REPORTS TO: EXECUTIVE DIRECTOR

The Programs & Community Outreach Coordinator is responsible for providing support to the clients and for providing leadership and coordination of the day-to-day programs. Responsible for developing relationships with local community groups and organizations to increase awareness of Hospice Georgina services.

DUTIES AND RESPONSIBILITIES:

  • Ensures the provision of cost-effective client services and programs.
  • Assesses clients for service needs, monitors service delivery and contributes to the development, evaluation and promotion of the program.
  • Develops and implements program plans to meet department needs. Manages volunteers and staff within the program, taking the lead in volunteer orientation, training and in-services within the framework of Hospice Georgina standards.
  • Increase engagement and awareness of Hospice Georgina.
  • Maintains regular engagement with clients and other health partners to ensure programs meet community needs.
  • Supports financial accountability within assigned programs by maintaining program budgets.
  • External and Internal Relationships: Timely communication and follow up with clients, and community partners/professionals as required. Timely communication and follow up with internal staff.
  • Ensure that clients understand the role of Hospice Georgina.
  • Ensure that a Service Agreement and consent form are completed and signed by the client.
  • Determine the appropriateness of all referrals and redirect appropriate referrals in a professional manner to other community resources.
  • Prepare and ensure accurate client data.
  • Process the necessary paperwork in the management of each case.
  • Ensure that all client and volunteer files are locked in the filing cabinet at the end of the day as per Hospice Georgina policy.
  • Support and advocate on behalf of a client as requested by the client.
  • Provide ongoing monitoring of clients and conduct client follow-up as needed.
  • Screen, provide orientation, train and place volunteers in appropriate placements.
  • Support volunteers with further training, skills development and resources.
  • Communicate with the volunteers and other members of the care team as needed.
  • Handle calls from volunteers concerning program issues.
  • Discuss any concerns and or complaints about a volunteer with the Executive Director.
  • Attend team-planning meetings when requested.
  • Provide reports and data as required and requested by the Ministry, Funders and the Executive Director.
  • Maintain a record of coordination activities and submit a summary report to the Board for review at monthly Board meetings.
  • Prepare an annual report of activities for the AGM
  • Provide a clean criminal reference check

Hours: Monday - Thursday 8:30-4:30pm

Some evening and weekend availability as needed
