Position Profile:

The Intense Youth Outreach Worker provides housing support, intensive case management and aftercare to youth ages 16-24. This position focuses on supporting youth who are experiencing homelessness, precariously, or unsheltered to find and sustain housing through intensive case management. This position supports youth in the shelters and community to find and sustain housing while developing and achieving goals from a client-centred and trauma-informed framework.


Role Overview

Responsibilities:

  • Provide housing support, case management, and aftercare services to youth

within the shelters

  • Help clients secure affordable and appropriate housing using a Housing First for Youth model
  • Assist youth in developing a comprehensive case plan, designed to concentrate on obstacles preventing success
  • Facilitate open communication with landlords/building superintendents to ensure positive relationships and resolve concerns
  • Build and maintain relationships with a broad spectrum of partner agencies, and community service agencies for referrals
  • Meet with or transport youth to appointments or locations in the community to assist in achieving goals
  • Facilitates programming for youth within the shelters and community
  • Provides community orientation to improve community integration by connecting with community services such as food, healthcare, education, counselling, legal aid, socialization, etc.
  • Coordination with financial supports to obtaining housing or prevent housing loss.
  • Compiles statistical information and prepares monthly reports as required for program evaluation and development.
  • Participates in committees and task forces with other agencies addressing issues facing youth
  • Participate in case management meetings, team meetings and meetings associated to care planning and organizational planning


Qualifications:

  • Diploma in Social Services or equivalent combination of education and/or experience.
  • 1-2 years experience working in community outreach supporting youth at risk of homelessness who face numerous barriers
  • Understanding the challenges experienced by youth in obtaining housing, employment, educational and community opportunities
  • Effective knowledge of youth agencies, resources and supports
  • Demonstrates working knowledge of housing first for youth framework, motivational interview, and trauma-informed support
  • Strong organizational, effective time management and planning skills, communication, advocacy, problem solving and negotiation skills
  • Excellent writing and documentation skills including the ability to research and complete applications and supporting documentation
  • Ability to work rotating shifts including some weekends and holidays
  • A valid Ontario Driver’s License, access to a vehicle and $2,000,000 (minimum) liability car insurance coverage are mandatory
  • Valid CPI/ UMAB and First Aid/ CPR certifications
  • Satisfactory Vulnerable Sector Police Check that meets the standards of Our Place Peel
  • Must be fully vaccinated against COVID-19, subject to applicable legislation
