JBH is a Clinical Education site in conjunction with McMaster University, and designated as an Academic Community Teaching Hospital. Its expanded campus includes the state-of-the-art Michael Lee-Chin & Family Patient Tower, featuring a new Emergency Department, 172 acute inpatient beds, 9 new Operating Rooms, a 20 bed Intensive Care Unit and a post-anaesthetic care unit to support expanded medical, surgical and outpatient services. JBH is also a partner member of the Burlington Ontario Health Team (OHT).

PACT (The Program for Assertive Community Treatment) is a specialized and intensive community outreach mental health service mandated to serve individuals with a severe and persistent mental illness residing in Burlington and Milton. Operating with its home-base at the Joseph Brant Community Health Centre adjacent to the hospital, the interdisciplinary team provides "round the clock" clinical treatment, rehabilitation, supportive services, and crisis response.

RESPONSIBILITIES:

  • Work one-to-one with youth to support them in the development and achievement of self-determination, personal empowerment, and their individual recovery goals
  • Support and working collaboration with youth, their families and the interdisciplinary team in the creation of recovery goals and or treatment plans
  • Collaborate with community partners, service providers and other stakeholders to help facilitate connections and referrals to other resources whenever appropriate
  • Share aspects of one's lived experience in a manner that is helpful to youth being supported, while demonstrating compassionate understanding and hope for recovery
  • Facilitate and coordinate youth focused peer driven/recovery oriented groups utilizing the values and principles of a peer support/person centred philosophy
  • Outreach to internal and external stakeholders to build awareness of and promote the Early Intervention Service to Youth
  • Maintain ethical and professional conduct in accordance with Psychosocial Rehabilitation Canada and Peer Support Accreditation Certification Canada
  • Perform appropriate record keeping and documentation for health record and statistical data entry

QUALIFICATIONS:

  • Minimum Grade 12 Diploma
  • Certificate in Peer Support Training from a recognized Training Provider
  • Formal training in Psychosocial Rehabilitation an asset
  • Demonstrated ability and understanding of core essentials of youth engagement and youth-focused peer support provision
  • Personal lived experience of mental illness and/or addiction
  • Demonstrated ability to role-model strong self-care and self-management strategies in recognition of the need for health, personal growth and resiliency in youth focused peer support work
  • ASIST Training an asset
  • Knowledge and experience working in a community based setting with young people experiencing serious mental health problems and/or addictions concerns
  • Knowledge of the unique developmental issues of adolescent/young adult clients and their families
  • Adaptable, creative, innovative and flexible
  • Excellent interpersonal, customer service skills; maintain confidentiality
  • Ability and willingness to travel and work in various settings/environments throughout the day (i.e. office, community, schools, work-places, homes).
