The work of the Downtown East Toronto OHT includes focused initiatives and planning to improve care for our Year 1 patient populations, ensuring lived experience partners are meaningfully part of all of our work, developing digital solutions, managing privacy and information management, strengthening governance, supporting performance and quality improvement, and developing a strategy for the OHT. In addition to this, the OHT is responsive to its environment, including responding to the COVID-19 pandemic.

The Project Manager and Advisor, Priority Populations will be responsible for guiding and supporting the work of the OHT overall, ensuring key deliverables and outcomes are achieved. The work of the OHT is supported by a team of project managers and analysts that form a secretariat. As the Director you, will lead the secretariat.

about the role:

The Downtown East Toronto Ontario Health Team (DET OHT) is a collaboration of 34 organizations who provide services to clients living in the Downtown East Toronto area. The purpose of the OHT is to provide an integrated health system but coordinating the care and services for patients and their families/caregivers across multiple settings. The vision of the DET OHT is to provide our community with seamless access to inclusive, high quality, and responsive care through one connected system of diverse providers and community members who enthusiastically embody holistic approaches to improving population and individual health and well-being.

The Project Manager and Advisor, Priority Populations, will work with Working Group Chairs, the Director of Operations and Manager of DET OHT Operations to ensure the successful completion of projects related to the DET OHT Priority Populations: Mental Health and Addictions and People Experiencing Homelessness. Responsibilities include all administrative and operational aspects, including defining and overseeing the scope, budget, and timelines of projects; applying project management principles; identifying and managing risks and issues; and completing reports as required by the OHT, Ministry or Ontario Health. In addition, the PM and Advisor will be required to engage stakeholders including but not limited to service provider organizations, community members, clinicians, DET OHT Partner Organizations (Team Members) and others.

day to day responsibilities:

The responsibilities of the Project Manager and Advisor, Priority Populations for the Downtown East Toronto OHT. includes:

PROJECT MANAGEMENT

  • Lead Working Group to develop and implement project plans
  • Conduct environmental scans as needed to determine scope of work
  • Use project management principles and tools to ensure success of project
  • Monitor projects
  • Identify and mitigate any risks to the any deliverables and the overall project
  • Work with operations team to ensure the project is leveraging and connected with other projects in the DET OHT
  • Ensure that project objectives and KPIs are aligned with DET OHT strategic plan and OH/Ministry deliverables
  • Create/complete reports and/or presentation to Core Group, other DET OHT WGs, Ministry, OH etc to communicate and demonstrate progress of projects
  • Ensure strong relationships among project partners including family and client advisors, OHTs, OH, community agencies etc, including engaging with new interested organizations on behalf of the working groups
  • Link with operations team, other DET OHT working groups and neighboring OHTs to stay aligned and leverage synergies
  • Assist working groups in establishing and adhering to a governance structure for the working group
  • Assist with creating/revising Terms of Reference
  • Ensure WG governance and ToR are aligned with the OHT Collective Decision Making Agreement
  • Work with communications network/specialist to develop communication strategy and messaging to stakeholders

Independent Decision Making

  • Oversee all aspects of the management of the project
  • Create and update project plan
  • Plan working group meetings including agenda and materials with WG Chairs

Consultative Decision Making

The incumbent will deal with highly complex situations for which several alternative courses of action are possible. In this case, the incumbent will make a decision by consulting with the Manager of Population Health and Operations and/or the Director of Operations

  • Finalization of the project plan
  • Identification and resolution of risks to deliverables

Ability to champion new initiatives, lead the change process, collaborate and form strong relationships across the OHT, and create an environment to achieve a desired change.
  • Organizational Planning: ability to lead streamlined processes to make them more efficient, and work with others to develop and adopt new procedures and processes. Participate and/or lead in management team meetings, special committees and working groups, as required.
  • Community Engagement: ability to foster and build strong relationships with community and funding partners, as required. Explore integrated and strategic partnerships that enhance services to the OHTs attributed population. Initiate evaluations, needs assessments, and surveys that assess emerging population needs/trends.
  • Commitment to cross-agency integrated program planning: ability to work co-operatively and collaboratively within the management team, working groups, or in regional/provincial work groups/ committees. This includes contributing fully and actively to team activities, projects, and program initiatives to achieve group and OHT goals. Resolve conflicts by facilitating the expression of diverse points of view to enhance teamwork and attain a beneficial resolution. Take steps to help others resolve the conflict and maintain trusting relationships. Promote co-operation between program teams.

To thrive in this role, you’ll need

education/experience:

  • Post-secondary degree/diploma in related field e.g., health administration, information technology, business administration.
  • 2-5 years of demonstrated leadership and management of complex multi-stakeholder projects, including but not limited to, system transformation, preferably in a hospital, community health or other healthcare setting.
  • Strong knowledge of and experience leveraging design thinking principles and lean methodology is an asset
  • Demonstrated experience leading collaborative initiatives involving multiple, diverse stakeholders.
  • Demonstrated excellence in interpersonal, written, and verbal communication skills including, ability to demonstrate tact and diplomacy, manage confidential information, and build rapport and positive working relationships
  • Demonstrated understanding of the health sector, including OHTs, seniors care, acute care, home care, long-term care, and community support services
  • Demonstrated advanced computer skills (Microsoft Office – Teams, PowerPoint, Word, Excel, Planner, Virtual Meetings – Zoom, Microsoft Teams)
  • Proven ability to work independently and as part of a team, demonstrating initiative and collaborative skills.
  • Exceptional organizational skills with the ability to manage multiple priorities

This position requires working in a fast-paced environment meeting the needs of multiple stakeholder. Time management, multi-tasking, initiative, organization and prioritizing are essential. Work is mainly performed in an office environment. Occasional work outside of normal working hours may be required, in order to meet with clients, vendors and partners.
