The Registered Nurse for the Home and Community Care program participates in assessments, interventions and care planning for clients in the remote community of Fox Lake, AB.

Clinical RN Responsibilities:

Receive referrals and perform assessments on new client admissions to Home Care. Complete client wellness checks and perform routine vital signs. Arrange wound, ostomy and continence assessments in collaboration with AHS, FNIHB Perform RAI assessments for referral to AHS sites for LTC, DSL or Respite. Provide Footcare and lower leg assessments. Prepare and complete case management for clients and families. Provide medication administration. Liaise with AHS physicians, pharmacists and Indigenous Services Canada staff to ensure Home care client are receiving timely access to urgent and emergency care. Provide Palliative care teaching to clients and families. Present ways in which to health teach clients about chronic conditions ie. HTN, diabetes. Assist Primary Care in emergencies when requested. Other clinical duties as requested.

Administrative:

Delegate tasks to other Home care team members, LPN and HCA'S. Order client specific supplies and medications. Assist client's with obtaining mobile equipment ie, wheelchairs, walkers, shower bars etc. Document client care in CARE and Chart in SOAP format. Enter new admissions and discharges in CARE.

Qualifications:

Current registration with CRNA in good standing. HCP Certification (Current). Basic and advanced foot care from an approved education institution. Clear vulnerable sector check with RCMP records check. Class 5 driver's license. Basic computer skills in Microsoft word, excel, powerpoint and Outlook. Fluent in the english language and the understanding/speaking of the cree language is highly preferred. Proactive, energetic and self-motivated candidate desired.

The successful candidate will be housed in a fully furnished residence and have the use of a fleet Home care vehicle. Basic utilities and maintenance of the unit covered by LRRCN.

Fox Lake is a picturesque Remote First Nation Indigenous Community with a population of approximately 2500 residents. It is located approximately 950 kms NE of Edmonton, AB and 120 kms East of High Level, AB. This small community has an elementary and junior/senior high school with a Northern Store Market and Gas station. Their is also an air strip, arena, daycare, college and RCMP station. In the winter months, the community can be accessed via the ice bridge which operates from Late December to mid March. In the spring, summer and fall, community members and the general public travel via the barge from Mid May to Late October to travel across the river to access other services.
