Position Summary

The Complex Support Coordinator (CSC) will provide coordination of the intensive supports needed for adults with developmental disabilities who have complex multisystem needs. These adults require a high level of active engagement and ongoing mentoring. The CSC will be responsible to provide a variety of supports including conducting assessments, coordinating the development of complex support plans, establishing linkages to services and coordinating cross-sector support for case resolution with the goal of improving the client’s access to supports and services, using an evidence-based, person-centered approach.

Hours of Work

Permanent Full-time (35 hours per week)

Flexible work hours (early mornings, evenings, and weekends) and work location/environment

  • Site flexible (Northern Ontario) – North Bay, Sturgeon Falls, Sundridge, Bracebridge, Parry Sound, or Timmins, ON (hybrid or primarily remote work may be considered)

Hourly Rate/Salary

Minimum $29.68 to maximum $37.19 per hour; Salary commensurate with education and experience

Qualifications

  • Bachelor degree in Social Work
  • Registered with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
  • Two (2) years related work experience required
  • Experience in the developmental disability sector is preferred
  • Knowledge of inter-agency and any other resources available for clients is an asset
  • Confident chairing meetings and making presentations
  • Strong organization, communication, and problem-solving skills
  • Team leadership and case coordination experience
  • Exceptional facilitation and collaboration skills
  • Excellent time management and organizational skills
  • Ability to work independently and as part of a team
  • Proficiency in the use of computers and a variety of software packages
  • Demonstrated commitment to workplace health and safety, with a willingness to contribute and lead in the culture of safety and prevention of adverse health events for clients and their families/caregivers, colleagues, other community members and yourself
  • Demonstrate ability and commitment to maintain confidentiality
  • Successful applicants will be required to provide:
    • proof of eligibility to work in Canada
    • a valid Ontario driver’s licence and use of a reliable motor vehicle; insurability on Agency vehicle insurance policy
    • clear Criminal Record Check, including Vulnerable Sector Search
  • Preference will be given to bilingual (French/English) candidates
