As a member of an interdisciplinary team, you will be required to provide psychosocial assessment, clinical interventions, consultation, discharge planning and educational services to frail elderly patients who may also have complex co-morbidities, and their families. Specialized Geriatric Services includes the Geriatric Rehabilitation Unit, the Day Hospital, the Musculoskeletal Rehabilitation Unit, the Regional Geriatric Outreach Team. The successful candidate may be required to work anywhere within the program.


Essential Qualifications

  • Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
  • Registered with the Ontario College of Social Workers and Social Service Workers
  • Masters of Social Work
  • Minimum 2 years experience in hospital social work with expertise in the care of the elderly, including dementia, psychogeriatrics and complex health conditions
  • Strong clinical knowledge and skills in comprehensive geriatric assessment, individual and family counselling, conflict resolution, discharge planning and advocacy
  • Knowledge and clinical experience in applying rehabilitative problem-solving, solution focused and brief therapeutic approaches to treatment and discharge planning with frail elderly and their families
  • Strong collaborative, communication, interpersonal, and problem solving skills
  • Knowledge of relevant legislation, including the Health Care Consent Act and Substitute Decisions Act
  • Demonstrated experience and competence in delivering the full range of clinical social work services with an outcome-oriented approach
  • Ability to execute early problem and needs identification through professional assessments and skillful implementation of all planned social work interventions in cooperation with the patient, family, and interdisciplinary team
  • Strong discharge planning skills
  • Comprehensive knowledge of and ability to mobilize community resources
  • Strong crisis intervention, advocacy, conflict management and facilitation skills
  • Highly developed interpersonal and problem-solving skills as well as the ability to function effectively independently and within a multi-disciplinary team
  • Demonstrated strength in written/verbal communication and case management
  • Ability to accept and positively influence change in the work environment
  • Excellent organizational, time management and communication
  • Demonstrated skills in providing education to patients, families and the health care team
  • Familiarity with outcome measurement related to clinical practice, ability to measure and evaluate practice change outcomes
  • Supports research and innovation in practice, previous experience in research an asset
  • Proficient computer skills with a variety of programs
Preferred Qualifications
  • Proficiency in French would be an asset

Immunization Requirements
  • Provide documentation you have received two doses of the Covid-19 vaccine and a written commitment to follow any future Public Health recommendations and St. Joseph's Health Care London policies and processes related to Covid management in the workplace.
  • Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox)
  • Provide documentation of the Tuberculosis skin testing
