Caregiver Program Coordinator

Lake Country Health (LCH) is looking for someone who is passionate about community and providing support to caregivers. LCH is a charity organization working to bring health and wellness services to the community.

Under the direction of the Executive Director, the Caregiver Program Coordinator is responsible for helping clients who are in a full-time caregiver role in Vernon, Kelowna, and Lake Country. With the aim to help them build skills and cope with their challenges, the role would provide services to help clients navigate health and social services, facilitate peer support groups, coordinate counselling services, and respite sessions. The Caregiver program is designed to support informal caregivers (family, friends, and neighbours) as they carry out their caregiving responsibility. This role in a hybrid position, working from home and an office in Lake Country.

The role facilitates peer support groups in Kelowna, Vernon, and Lake Country; a reliable vehicle is needed for this position. On average, the position requires 33 hours/week.

Duties and Responsibilities:

  • Maintain client confidentiality and adhere to PIPA requirements at all times
  • Facilitate, coordinate, and schedule caregiver support
  • Coordinate a volunteer respite services
  • Coordinate events and presentations with outside agencies on caregiver topics
  • Provide systems navigation, work with individual caregivers on locating, obtaining, and using available health and human services
  • Provide counsel and support to individual caregivers to assist in making decisions and solving problems related to their caregiving roles
  • Assess and support system navigation for respite care and supplemental services
  • Reporting and data entry, track services
  • Gather, develop and maintain caregiver resources
  • Responsible for obtaining contracts for services and arranging for payment
  • Attend training as required
  • Perform other job-related duties as required

Qualifications:

  • Must have excellent verbal and written communication skills and proven interviewing and counselling skills.
  • Must be able to manage several demands at once and work independently.
  • Excellent organizational skills.
  • Knowledge of social services or human services issues, especially working with older adults and family caregivers.
  • Ability to work within a fast-paced, team-based nonprofit office.

Education & Experience:

  • Bachelors degree in behavioural or human services or diploma in behavioural or human services supplemented by courses specialized in gerontology.
  • Experience working with dementia, including Alzheimer’s and other ageing illnesses.
  • Proven program coordination skills and understanding of how health and human services operate in BC.

Other Skills & Abilities:

  • Current CPR and First Aid
  • Recent experience with dementia or memory care background is an asset.
  • Computer skills required (proficient in word, excel and access) including data management.

This position requires a current and clear Criminal Record Check Form and Clearance Letter prior to the hire date. Must also possess and maintain a valid driver’s lice

Salary: $26.00-$28.00 per hour
