The Opportunity:

We are recruiting a Nurse Clinician RN (Specialized Geriatric Service)

  • This is a regular full-time position (35 hours per week Monday to Friday 9am to 5pm) based at 234 St. Patrick St. E. Fergus with travel throughout Wellington county.

As an outreach based Specialized Geriatric Services nurse clinician who is person-centered, team oriented and collaborative in your approach, you will work together with seniors and their care partners living with chronic disease, complex mental health and/or addiction issues and/or dementia to optimize functioning and quality of life utilizing findings from comprehensive mental health assessment and consultation. The Nurse Clinician provides in-home/outreach assessment, care planning, follow up and consultative services for individuals with complex geriatric mental health concerns and medical complexities. This role has an advanced ability to identify risk and apply appropriate mitigation strategies, and requires a solid understanding of what factors contribute to complex client presentations (e.g. adverse childhood experiences, developmental trauma, attachment and concurrent disorders). The Nurse Clinician works closely with the geriatric system partners including Primary Care, Geriatric Psychiatry, Long Term Care and LHIN Care Coordinators. This position reports to the Manager, Seniors Service.

Qualifications:

  • Diploma/degree from an approved nursing program. Current registration and good standing with the College of Nurses of Ontario and eligibility to practice as a Registered Nurse in Ontario
  • 3-5 years of experience in the provision of services to a geriatric population with medical, cognitive, mental health, addictions, and behavioural complexities
  • Skills, knowledge and clinical practice commensurate with the administration and interpretation of Geriatric Mental Health assessment, consultation, care planning implementation and intervention functions
  • Experience in geriatric medicine an asset
  • Excellent interpersonal skills and familiarity working in a team environment
  • Excellent organizational, problem solving and documentation skills utilized in the management of an active client caseload
  • Demonstrated understanding and sensitivity to social and cultural diversity
  • The ability to work collaboratively with formal and informal community partners
  • Proficiency with MS Office, Outlook and various database software
  • Valid driver’s license, vehicle and insurance, as travel is required
