Reporting to the Operations Manager of the Mobile. Medical. Support. (MMS) unit, the Nurse Practitioner will assist with the responsibilities of mobile community outreach across Windsor Essex. The MMS team is deployed out to High Priority Communities to provide interdisciplinary Primary Care and social supports. They will work either on the mobile MMS unit or indoors at partnering agencies with the client population, nursing team, and partners. They will also provide virtual support to their team members at tandem clinic locations as needed and will attend community events. The commitment to this position is 75 hours per biweekly pay period. The successful applicant will be required to work the following shifts: 8:30am-4:30pm, 10am-6pm, and 12pm-8pm.


Responsibilities
  • Facilitate and implement systems, which promote and maintain effective communication and collaboration with physicians in the community, nurses, other members of the health care team and the community service providers
  • Develop primary Health Care and Health Promotion programs under the direction of the A.E.D. Patient Services for the identified community partners
  • Collaborate with physicians and members of M.M.S. in the development of Medical Directives for the delivery of patient care
  • Consult with Physicians according to the CNO’s document, Standards for Practice – RN (EC), 1988
  • Collaborate with community partners and patient advocacy groups to identify needs and to develop primary care programs that are culturally appropriate
Knowledge, Skills, and Abilities
  • Identify the need for and participate in scholarly/research activities
  • Maintain and enhance a current level of knowledge relative to professional practice as well as continuing education requirements necessary for licensure
  • Coach and teach staff in the health care management of selected patient populations, both formally and informally
  • Assist staff as appropriate in the development of clinical decision making and problem solving skills through case consultation, clinical demonstration, interviewing, case reviews, etc.
  • May precept postgraduate students from various programs
  • Contribute to the education of nurses and other health professionals through teaching, publication and presentations
  • Facilitate own professional development and maintain clinical competency through formal educational programs and mentoring opportunities with consultant(s) and peers
  • Develops and participates in quality-monitoring programs for the services and routes results to service managers
  • Conduct needs assessments
  • Participate in the development of programs and services geared towards community outreach
  • Participate in the program evaluation process and exhibit strong personal commitment to continuous quality improvement
  • Maintain appropriate records and documentation of clinical and educational activities
Leadership
  • Provide leadership in the development and revision of scientifically based clinical standards for a selected patient population
  • Demonstrate leadership within clinical team and with community partners to support strategic direction and planning
  • Monitor compliance with clinical standards
  • Demonstrate leadership skills, expertise, and an in-depth knowledge base consistent with advanced nursing practice
  • Identify need for change to clinical, education and research issues in nursing practice

  • Practice according to the six competency areas defined by the Colleges of Nurses of Ontario
  • Health Assessment and Diagnosis – performs a comprehensive health/symptom directed assessment and synthesized data from multiple sources to make a diagnosis of disease or disorder within the PHCNP scope of practice to diagnose
  • Therapeutics (including pharmacological, complementary and counseling interventions) – initiates and manages the care of clients with disease or disorders within the PHCNP scope of practice and/or monitors the ongoing therapy of those patients with chronic stable illness by providing effective pharmacological, complementary or counseling interventions
  • Role and Responsibility – promotes and participates in an interdisciplinary provider model
  • Health Promotion and Disease Prevention – implements strategies to promote health and prevent disease with individuals, families and communities
  • Family Health – synthesizes information from individual clients to identify broader implications for health within the family
  • Community Development and Planning – utilizes community assessment data to identify community needs and resources and develops, implements, and evaluates appropriate programming in collaboration with other providers
Qualifications/Skills Required
  • Current certification of competence as an RN in Ontario
  • Registered with the College of Nurses on Ontario in the Extended Class – Nurse Practitioner, with specialization in Primary Care
  • Knowledge of Nurse Practitioner’s prescribing guidelines as set out by the College of Nurses of Ontario
  • Maintains and enhances a current level of knowledge relative to professional practice as well as continuing education requirements necessary for licensure
  • Responsible for advancing skills and education by: seeking learning experiences and responsibilities to strengthen areas requiring development, assessing own performance status in relation to this position description, and setting realistic and measurable goals for individual development
  • 5 years’ experience Primary Care and Community
  • Triage training and experience
  • Program planning and development skills in collaboration with Community Partners
  • Certificate in Primary Care for Nurse Practitioner preferred
