Job Summary

STADD is currently a regionally based program with a mandate to provide a one-government integrated transition planning model. STADD’s current mandate is to support youth and young adults ages 16-24 with intellectual disabilities, autism spectrum disorder, and/or fetal alcohol spectrum disorder, who are eligible for CLBC at adulthood as they transition to adulthood and/or across service systems.

Under Budget 2022, the Ministry announced the creation of a new Youth Transitions Program to support youth and young adults in and from care aged 14 to 27 with new and enhanced services and benefits. STADD and the Agreements with Young Adults (AYA) program will become part of the overall Youth Transitions Program, which includes the introduction of funding to better support youth and young adults as they transition from care, expanded program eligibility and duration, and the creation of post-19 transition coordinators to help young adults up until the age of 25 navigate their transition and move toward independence more successfully.

Under the supervision of the Team Leader, the Transition Support Worker is a key point of contact and resource for young adults who have left the care of the ministry. Transition Support Workers support young adults in their transition to adulthood by providing continuity of service, a connection to a broad range of adult government, community and cultural supports and services, and access to financial supports post 19 years of age. The Transition Support Worker supports Navigators to ensure the right people are brought together to collectively plan, make decisions, and commit to actions that best support the transition of young adults to adulthood. Transition Support Workers collaborate with other ministry staff to support relational and cultural stability, assist young adults to consider and plan for their future goals, and support the management and updating of the transition plan.

Transition Support Workers are part of provincial and regional teams that are not all physically co-located together. They may be expected to work independently due to their geographic location. STADD teams use virtual tools to connect for tele- practice meetings and share information in real time. This includes phone, videoconferencing, computer technologies as well as participating in in-person meetings with young adults and partners in community locations as feasible or required.

(As part of the provincial implementation of the Youth Transitions Program, STADD and AYA program names and worker titles will be rebranded and will change).

Job Requirements:

  • Degree or related diploma in an identified human services field (Psychology, Sociology, Criminology, Anthropology, Early Childhood Education, Theology, Indigenous Studies, Education, or Nursing).
  • Minimum of 3 years of combined experience working with vulnerable youth and young adults with complex needs in the following areas:
    • Working in the community with youth or young adults with intellectual disabilities, Autism Spectrum Disorder or Fetal Alcohol Spectrum Disorder, and their families; AND
    • Working with vulnerable youth and young adults with complex needs (i.e., substance use, mental illness, homelessness, or youth in or formerly from ministry care).
  • Other combinations of related education and directly related experience may be considered.
  • Demonstrated experience in the use of virtual technology (MS Teams, Skype, Zoom, etc.) to deliver services and sustain meaningful engagement with youth, young adults, and families.

NOTE: All education credentials will be subject to verification as per the outlined ministry process. If this education was obtained outside of Canada, you need to confirm it has been assessed for equivalency through the International Credential Evaluation Services (ICES). You must provide a comprehensive report and the MCFD supplemental report by the closing date of this posting to be considered.

Preference may be given to applicants with 1 or more of the following:

  • Preference may be given to candidates with lived experience as a youth in care with the Ministry
  • Preference may be given to applicants with experience working with youth or young adults in and from the care of MCFD or indigenous Child and Family Service Agencies (formerly known as Delegated Aboriginal Agencies).
  • Preference may be given to applicants with experience in planning, coordinating and hosting presentations using traditional and modern training methodologies.
  • Preference may be given to applicants with experience in successfully engaging Indigenous children, youth, families, communities and other Indigenous partners.
  • Preference may be given to applicants who voluntarily self-identify as Indigenous.

Willingness Statements:

  • Must possess and maintain a valid BC (full) class 5 driver’s license.
  • May be required to work flexible hours (work evenings/weekends).
  • May be required to use own vehicle (appropriate insurance coverage) on an expense account basis.
  • May be required to travel in remote locations.
  • May be required to travel in all (reasonable) weather conditions.
  • Willingness to work with youth, young adults, and families facing challenges which may occasionally involve exposure to situations involving individuals in crisis or requiring specialized supports.
