Position Description: This position, under the direction of SVRS Supervisor, Program Manager and Director, requires a flexible work schedule to meet the resident’s needs. This will include day, evening, overnight awake and overnight sleeper shifts (weekdays and weekends). Key worker should be able to work these on short notice. In emergency situations there may be a requirement to stay past the scheduled shift completion time.

Responsibilities and Qualifications

The Key worker (2 key client) will be responsible for the following:

· Provide individualized case management and program services to individuals in care with SVRS.

· Work as a member of SVRS ensuring the health and safety of the individuals in her/her care. Ensuring adherence to health and safety standards and expedient reporting to Supervisor Manager or a Director of potential hazards.

· The ability to act effectively in situations demanding prompt assessment and immediate actions.

· Attend all training sessions as required

· Work in partnership with a multidisciplinary team to aid in the development and the delivery of programs and strategies.

· Follow the established practices and procedures of the residence and carry out appropriate action in handling situations.

· Completing day to day administrative tasks including the completion of the daily logs and other assigned reports or tasks, communicate effectively through recordings, at shift change and all other meetings.

· Performing and/or supervising individuals in care in the performance of assigned household chores.

· Being familiar with and implementing any specific management and activity plans.

· Acting as in-charge staff as scheduled

· Driving residents in care to work, school, day programs, home visits and appointments or to special activities or events.

· Maintaining a professional attitude in all matters pertaining to the work of the residence/agency

· Safeguard the legal rights, privacy, and health of the residents by acting as their key worker.

· Attend and chair case conferences in the role of Key worker

· Develop an in-depth knowledge of assigned residents in care regarding their developmental, physical, intellectual, communicative, emotional, social, and spiritual needs.

· Liaison with school, parents/guardians, and other service professionals to provide comprehensive care to key residents

· Program development with the Clinical supervisor: carrying out individual assessments, program plans or other therapy in accordance with structured programs as established by the Clinical Supervisor.

· Analysis of behavioural data including the graphing of specific data.

· Responsible for the writing and revisions of Plans of Care for each key resident.

· Ensure that the residents are safe and that all their physical and emotional needs are met.

· Ensuring appropriate medication is given and taken by assigned residents in care, as prescribed and ensure constant monitoring for the possible side effects and behavioural effectiveness in consultation with medical practitioners, Clinical Psychological Associate, Supervisor, Manager and Directors.

· Administration of First Aid as required by individuals in care of staff in the day-to-day operation of the residence.

· Monitoring daily monies and personal expenditures of residents in care.

· Follow the established practices and procedures of the residence and carry out appropriate action in handling situations such as runaways, illness or accidents, fire alarms, fire, scheduled or unscheduled visits, episodes of upset, disclosure and “acting out behaviours”

· Assessing progress and maintaining appropriate documentation required by the residence, other agencies or government Ministries.

· Completing day to day administrative tasks including the completion of the daily logs and other assigned reports or tasks, communicate effectively through recordings, at shift change and all other meetings.

· Performing and/or supervising individuals in care in the performance of assigned household chores. (such as cooking, dishwashing, floor washing etc.)

· Planning menus, food purchasing, food preparation and implement as scheduled.

· Being familiar with and implementing any specific management and activity plans.

· Check communication book for appointments ensuring that all residents’ appointments are kept and record further appointments.

· Adhering to scheduled programming, planning and developing new programming aspects, caring out shift routines.

· Organizing or co-ordination of special events, recreation or physical fitness programs etc. for individuals in care, both individually and in groups.

· Ensuring adherence to health and safety standards and expedient reporting to Supervisor, Manager or a Director of potential hazards.

· Scheduling fire drills and fire equipment safety checks when requested by a Supervisor, Manager or a Director.

· Scheduling vehicle maintenance when requested, ensuring appropriate procedures are followed and vehicle problems or potential problems are reported immediately to a Supervisor, Manager or a Director.

· Modifying shift schedules as directed by as Supervisor, Manager, or a Director.

· Participate in meetings, conferences and training seminars as required.

· Supervise students or field placements

· Driving residents in care to work, school, day programs, home visits and appointments or to special activities or events.

· Establish constructive professional working relationships with clients, staff members or the residence and the agency.

· Keep Supervisor, Manager and/or Director informed of activities and problems which may have important implications for residents, their families, and Springview as an agency.

· Ensuring the general well-being and direct care of the residents, providing a constant high level of supervision.

Note: Springview’s clients may at times exhibit difficult to manage behaviors such as noncompliance, self injury, destructive and aggressive behavior. Even though Springview creates empirically based individualized clinical programming to assist in addressing these behaviors, it still remains impossible for Springview to accurately predict and be responsible for the actions of its clients.

Qualifications: This position requires maturity, proven commitment to working with individuals in care who posses a range of developmental disabilities and behavioral challenges, excellent oral and written communication skills, case management skills, solid assessment, program planning, implementation and monitoring skills, enthusiasm, working knowledge of behavioral management methods and social work principal/practices, special education practices, excellent social skills. To have or be able to acquire skills in the area of teamwork, leadership, medication administration, report writing, Therapeutic Crisis Intervention and/or Crisis Prevention Intervention and administrative efficiency.

Enrolment and the completion of at least one year of study in the Developmental Services Worker program or a related program from a recognized community college/university

Six months experience working with clients with high behavioural needs, preferably persons with disabilities.

Consideration will also be given to those without college or university education, who have a minimum of two years work experience with individuals with developmental disabilities.
