The Registered Nurse - Emergency Department Transition Coordinator (EDTC) is a clinical consult-based position. EDTCs assess patient needs in the context of the whole person including their life situation and determinants of health. Critical thinking and accurate interpretation of complex information from a variety of sources are essential skills for this position. The role of the EDTC is highly complex and includes comprehensive assessment, planning and coordination of care, as well as ongoing communication with other health-care providers across the continuum of care. The successful candidate must be able to problem solve and cope with fluctuating priorities and time sensitive deadlines. Conflict resolution and strong communication skills are required. Major responsibilities include: completing comprehensive needs assessments, determining urgency/priority, developing and implementing appropriate service plans, coordinating service delivery across programs/settings, liaising with Continuing Care staff and other community resources, providing information and support to clients, families, and caregivers. The EDTC works within a multi-disciplinary team of health professionals.

Description:

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $35.17/hour - $42.96/hour ($35.87/hour - $43.82/hour effective April 1, 2023). Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable. Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).

Additional Required Qualifications:

A minimum of 3 years Acute Care based nursing experience including geriatric experience (obtained within the last five years) is required. Effective time management and prioritization skills are required. Conflict resolution and strong communication skills are required. Must be able to work independently and within a team environment. Ability to problem solve and demonstrate critical thinking skills is required. Demonstrated ability to provide comprehensive assessments to individuals within their physical and social environments is required. Must be able to cope with high levels of stress, fluctuating priorities, and time sensitive deadlines. Knowledge and ability to use computer applications such as Microsoft Office, Excel, and Outlook is required. Work tasks and charting is computer based so there are extensive repetitive motion tasks. Must be able to tolerate long periods of sitting, or standing as well as repetitive movements. Must be able to participate in hands on care of patients (i.e., transfers, bariatric clients, bending, Reaching, twisting, and walking distances). Must be able to lift and carry 10-15 kg regularly if needed.

Preferred Qualifications:

Experience as an Acute Care Coordinator or Transition Coordinator is preferred. Experience in home care, supportive living or facility living an asset. Experience with Connect Care is preferred. Experience working in the ER preferred. Experience with Meditech and RAI Preferred.
