Position Summary:
Under the direction of the Program Manager and working as a member of the Palliative Care Program, the Grief and Bereavement Coordinator works with the Program Coordinator and other members of the Palliative Care team on the design, coordination and delivery of grief and bereavement services and programs for Palliative Care clients and their families.
This role supports clients through the process of grieving their loss. The role is tasked with implementing and maintaining a program that includes psycho/social assessments and provides personal counselling in person, by telephone, or by videoconference. This support may be provided individually or in groups to brereaved and/or grieving individuals referred to the Palliative Care Program. This position requires the ability to work with a high degree of independence with skills in multi-taksing.
Participates in and/or provides in-service education and hospital activities that model and promote a safe and healthy, patient-centered environment and that contribute to operational efficiency, professional growth, and program development.
